Dr. Nancy Beran: Innovators Awards 2024

Beran, an internist, works to identify at-risk populations, decrease inequities, and improve patient access to care at New Hyde Park, New York-based Northwell, which has 21 hospitals and some 900 outpatient facilities. She designed the system’s Ambulatory Quality Improvement Collaborative framework, which seeks to address chronic disease through a population health lens with the guidance of an executive oversight committee and a physician-led multi-disciplinary workgroup. Through the framework, the team creates patient care standards, implementation strategies and communication plans. The AQIC led to improved outcomes for patients with hypertension and diabetes in its first year.
